The lack of equitable access to cutting-edge high-tech medical treatments can perpetuate and worsen existing inequalities in healthcare. Analyzing US hospitals that either established or avoided implementing left atrial appendage occlusion (LAAO) programs, the characteristics of their patient populations, and the associations between zip code-level racial, ethnic, and socioeconomic demographics and LAAO rates among Medicare recipients in expansive metropolitan areas with LAAO programs. Between 2016 and 2019, we performed cross-sectional analyses on Medicare fee-for-service claims for beneficiaries aged 66 years or above. The study period revealed hospitals that implemented LAAO programs. Employing generalized linear mixed models, we investigated the correlation between age-adjusted LAAO rates and the racial, ethnic, and socioeconomic makeup of zip codes in the 25 most populated metropolitan areas with LAAO facilities. During the period of observation, 507 candidate hospitals started LAAO programs; in comparison, 745 hospitals did not embark on these programs. A substantial 97.4% of newly opened LAAO programs were positioned within metropolitan areas. A statistically significant difference (P=0.001) was observed in the median household income of patients treated at LAAO centers compared to those treated at non-LAAO centers, with LAAO centers having $913 higher income (95% CI, $197-$1629). The impact of concurrent corticosteroid use on the effectiveness of immune checkpoint inhibitors (ICIs) for melanoma brain metastases (MBM) is indeterminate.
